Employees may elect to enroll in the Guardian group dental plan offered.
There is an employee premium co-pay based on salary.

• $100,000 or more in salary pays 40% of the premium.
• $60,000 to $100,000 in salary pays 35% of the premium.
• $60,000 or less in salary pays 30% of the premium.

A pre-tax premium payment option is available.
